Carolyn Barrera Lactation Consulting is committed to preserving your right to privacy, and protection of your personal and health information in accordance with PIPEDA and HIPPA legislation. It’s important that you know what type of information may be collected about you, and how your information is used, shared, and stored.

Personal and health information collected may include, but may not be limited to, you and your baby’s names, dates of birth, address, phone number, email address, and health histories, as well as any further information you voluntarily disclose. Any personal or health information you provide, via the Contact form or other means, will only be collected, used and shared for the following purposes:

1) Contacting you to arrange the provision of the care you have inquired about or requested.
2) Developing a mutually agreeable care plan based upon your stated goals.
3) Providing a 3rd party such as your primary care provider, referring physician, nurse practitioner or midwife, and/or insurance provider, with information to assist in providing appropriate care and follow up.
4) As required by law.

Carolyn Barrera does not sell your personal information to any 3rd party.

Appropriate and reasonable technical, physical and administrative security measures are taken to safeguard your personal information from such risks as unauthorized access, collection, use, disclosure, copying, modification, disposal or destruction.Your personal information will be used and retained only to the extent and duration necessary to administer your care and comply with our legal obligations.

No data storage nor transmission over the internet and mobile devices can be guaranteed to be 100% secure. There is always some risk when transmitting information to us through the website Contact form and by other electronic communication devices.

Virtual consultations are delivered via a HIPAA/PIPEDA compliant platform. To protect your privacy, Carolyn will remain in a private and secure location for the duration of the consultation.

You have the right to seek access to your health records that we keep and to ask us to correct a record if you believe it is inaccurate or incomplete.

You have the right to request a withdrawal or suspension of consent to the use of your personal and health information, which will be honoured unless legal obligations prevent us from doing so.
